Pursuant to Executive Order (EO) No. 139 dated August 18, 2021, amending EO 115 dated August 5, 2002, barangay officials who died during their term of office are covered by death benefit claims nationwide.
Beneficiaries of a deceased Punong Barangay can receive P20,000 for death benefit and P2,000 for burial expenses.
Beneficiaries of a deceased member of a Sangguniang Barangay, Sangguniang Kabataan (SK) chairpersons as ex-officio members, barangay secretary, barangay treasurer and Indigenous People Mandatory Representative (IPMR), can receive P10,000 death benefit and P2,000 for burial expenses.
IMPORTANT NOTE: Before proceeding to Client Step 1, please ensure that you have complete documentary requirements.
✅ DOCUMENTARY REQUIREMENTS:
Oath of Office
Certificate of Assumption to Office (applicable to 2023-2025 term of office)
Certification that the barangay official was incumbent at the time of his/her death (issued by the DILG C/MLGOO or Punong Barangay)
Certified true copy of the death certificate of the deceased barangay official
Photocopy of any valid government-issued ID of the claimant
✅ Claimant's documents, whichever is applicable:
a. Marriage Certificate, if the claimant is the widow/widower of the deceased barangay official
b. Birth Certificate of the deceased barangay official, if the claimant is the parent of the deceased barangay official
c. Birth Certificate of the claimant, if son/daughter or brother/sister of the deceased barangay official
d. A Special Power of Attorney, which shall be executed as supporting document in the case of multiple designated beneficiaries/claimants of deceased barangay officials, except if the claimant is spouse. Only one check shall be issued to the person whom the beneficiaries agreed from among themselves to receive the check.
e. A designated beneficiary/legal heir who is a minor shall be represented by a guardian. An affidavit of Guardianship shall be executed for the purpose
f. In the absence of primary and secondary beneficiaries/ claimants, an affidavit of two (2) interested persons or affidavit of self-adjudication shall be executed by the claimant to support that he/she is the only heir of the deceased barangay official
g. If the claimant is the Common-Law Spouse/Parent of the deceased barangay official, the legal heir shall execute an Affidavit or Special Power of Attorney
✅ Other supporting documents:
h. Photocopy of Police Report, in case the matter of death of the deceased barangay official is not a natural cause
i. Affidavit of two (2) disinterested parties shall be executed if there is a discrepancy in the name of the deceased barangay official or claimant
j. For beneficiaries of IPMRs, if applicable, in the absence of the documents mentioned above, affidavit of two (2) disinterested persons shall be required

✅ Client Steps
STEP 1: Please download and accomplish the Citizen’s Charter Request Form (CCSRF).
STEP 2: Kindly submit the filled-out Citizen’s Charter Request Form (CCSRF) and other documentary requirements either thru email dilgtacurongcity@gmail.com, personally, or via courier to the DILG Tacurong Field Office (DILG Tacurong City, 2nd Floor, City Government Hall, Brgy. Población, Tacurong City, Province of Sultan Kudarat 9800).
STEP 3: Please wait for the notification which will be sent through email / SMS / call acknowledging the receipt of your request and providing other updates. You will also be notified once your requested document is ready for release.
STEP 4. Once you have claimed/received your requested document, kindly fill out the Client Satisfaction Survey Form (CCSF).
